Use this item to pay for any number of FOA certifications - choose the proper quantity.
Only one certification type (e.g. CFOT or AFOT) per order. Order another "product" for a different certification type.
Each order must identify the CFOT numbers assigned to the students included in this payment. Putting in either individual numbers or a range (e.g. xxx1200-xxx1215) is acceptable.
Student applications must be filed separately by fax, email or postal mail.
Please print a copy of this order and include it with the student applications if mailed or faxed. If emailing student data, include this order number with the email.
